Friend's PS4 stuck in Safe Mode?

I've only ever done something like this once, flashing the firmware onto a new internal hdd years and years back.

If I recall correctly Sony offers 2x firmware files, one bigger than the other. Think it was update va update+firmware or something. Is it possible he's got the wrong install file?

Anyone know how to get him un-stuck from Safe Mode?

If your PS4 (All models) is stuck in SAFE MODE, here's the fix.
You may need a USB stick (8GB best) and the PS4 Offline Update File, links below.

Update from friend:

"Put the update on the stick and ps4 told me it has completed the 10.01 update. So, I've managed to successfully do the update I think, but the ps4 takes me straight back into bloody safe mode!

Asking this but I've just successfully installed it. Could it be a sign the internal hard drive is dead or bad health? Lol"

That sounds strange to me? Shouldn't the PS4 be booting normally now if the firmware's had on-console installation confirmation?
